dc.description.abstract | Introducción: La insuficiencia cardíaca actualmente es un problema de salud pública mundial
asociado a una morbilidad y mortalidad significativa. El déficit de hierro es la deficiencia
nutricional más común en estos pacientes, contribuye tanto al empeoramiento clínico así como al de
la fracción de eyección del ventrículo izquierdo. Siendo importante estudiar la relación entre ambas
patologías y tratar esta comorbilidad para tener mejores resultados. Objetivo: Determinar si existe
relación entre el déficit de hierro y la limitación funcional y fracción de eyección en la insuficiencia
cardíaca en el Hospital Nacional Adolfo Guevara Velasco, agosto - noviembre 2023. Metodología:
Estudio cuantitativo, correlacional, observacional, prospectivo y transversal. La muestra fue por
conveniencia, participaron un total de 100 pacientes, quienes cumplían con los criterios de
inclusión. Se calculó la significancia mediante la prueba estadística de

Resultados: De los 100 pacientes estudiados, el grupo de edad ≥ 60 años representó el 70%, el 52%
fue de sexo masculino y el tipo de atención médica con mayor porcentaje fue la hospitalaria (56%).
En cuanto a la valoración clínica de la capacidad funcional (NYHA), las clases III y IV fueron en
mayor proporción (65%) frente a las clases I y II (35%). Los pacientes con FEVI reducida fueron la
mayoría (52%), seguidos de la preservada (33%) y ligeramente reducida (15%). El déficit de hierro,
fue determinado según la ferritina sérica, donde el 65% la tenían < 100 ng/ml (déficit de hierro
absoluto), no se contó con la saturación de transferrina para el diagnóstico de DH funcional. Se usó
la hemoglobina como indicador de fase avanzada de déficit de hierro, donde el 30% del total de
pacientes tenían este parámetro por debajo de los valores normales. Se encontró que existe
asociación entre el déficit de hierro y la clasificación funcional III y IV según NYHA (p <0.001),
así mismo, existe asociación entre la deficiencia de hierro y la FEVI reducida (p<0.001).
Conclusiones: Sí existe relación entre el déficit de hierro absoluto y las limitaciones funcionales
clase III y IV según NYHA. También existe relación entre el déficit de hierro absoluto y la fracción
de eyección reducida en pacientes con IC en el Hospital Nacional Adolfo Guevara Velasco, agosto -

Results: Of the 100 patients studied, the age group ≥ 60 years represented 70%, 52% were male and
the type of medical care with the highest percentage was hospital (56%). Regarding the clinical
evaluation of functional capacity (NYHA), classes III and IV were in a greater proportion (65%)
compared to classes I and II (35%). Patients with reduced LVEF were the majority (52%), followed
by preserved (33%) and slightly reduced (15%). Iron deficiency was determined according to serum
ferritin, where 65% had it < 100 ng/ml (absolute iron deficiency), transferrin saturation was not
counted for the diagnosis of functional ID. Hemoglobin was used as an indicator of advanced phase
of iron deficiency, where 30% of all patients had this parameter below normal values. It was found
that there is an association between iron deficiency and functional classification III and IV
according to NYHA (p <0.001), likewise, there is an association between iron deficiency and
reduced LVEF (p <0.001). Conclusions: There is a relationship between absolute iron deficiency
and class III and IV functional limitations according to NYHA. There is also a relationship between
absolute iron deficiency and reduced ejection fraction in patients with HF at the Adolfo Guevara
