dc.description.abstract | Introducción: La leucemia linfoblástica aguda es la neoplasia pediátrica más frecuente en nuestro
medio y se da como resultado de una proliferación anormal de células hematopoyéticas
progenitoras que generan alteraciones en las distintas series celulares.
Objetivos: Determinar el perfil epidemiológico, clínico y laboratorial al debut de los pacientes
menores de 15 años con diagnóstico de leucemia linfoblástica aguda en los tres principales
hospitales de la ciudad del Cusco durante los años 2018 y 2023.
Materiales y métodos: Estudio de tipo no experimental, descriptivo, transversal, observacional
y retrospectivo. Se revisaron 92 historias clínicas físicas y digitalizadas en los tres hospitales
cusqueños de pacientes menores de 15 años con diagnóstico confirmado de leucemia linfoblástica
aguda, se buscaron los datos al debut, obteniendo información que fue ordenada y registrada
mediante una ficha de recolección de datos; los resultados obtenidos se presentaron en tablas
expresadas en frecuencias absolutas y porcentajes.
Resultados: La casuística de pacientes pediátricos que debutaron cada año con leucemia linfática
aguda entre el 2018 y 2023 fue de 14 casos por año. Dentro de las características epidemiológicas
más importantes tenemos que el sexo masculino fue el más afectado (62%), así también los grupos
etarios más registrados fueron el de 1 a 5 años y el de 11 a 15 años con el (62%) y (40%)
respectivamente. El lugar de donde los pacientes provenían con mayor frecuencia fue de
provincias (48%), siendo la Convención la provincia con mayor cantidad de casos de este grupo
(23%). Los síntomas más frecuentes al debut fueron palidez (92%), astenia/adinamia (80%),
malestar general (79%), sangrado/hemorragia (45%). Al examen físico el hallazgo más frecuente
al debut fueron las adenopatías (70%) de las cuales las cervicales representaron el (81%). A nivel
laboratorial se encontró que el 57% debuto con anemia severa, de los cuales el 73% fue de tipo
normocítica/normocrómica, a nivel del recuento plaquetario se evidencio que el 33% debuto con
menos de cincuenta mil plaquetas, el 63% de casos debutaron con linfocitosis, el 43% debutaron
con leucopenia y 36% con neutropenia severa. El LDH se encontró por encima de 500 U/I en el
58% de pacientes, así como también se encontró que el 25% de pacientes presento niveles de
ácido úrico por encima de 8mg/dl. La estirpe de leucemia más frecuente fue la de tipo B con el
91%, siendo el sub tipo B común el más frecuente de estos con el 78%, el cariotipo más frecuente
fue el diploide (normal) 58% y en el panel molecular en el 79% de los casos no se encontraron
alteraciones al debut.
Conclusiones: En las características epidemiológicas se determinó que esta enfermedad es mas
frecuente en varones, que la edad de debut predilecta para esta neoplasia es entre los 1 a 5 años,
y que esta enfermedad es más frecuente en provincias. Clínicamente debutan con palidez y
adenopatías. En los hallazgos de laboratorio la mayoría de pacientes debuta con anemia severa,
leucopenia, y trombocitopenia severa, así como también el inmunofenotipo B, el cariotipo
diploide y el panel molecular sin alteraciones fueron los más frecuentes al debut. | es_PE |
dc.description.abstract | Introduction: Acute lymphoblastic leukemia is the most common pediatric neoplasm in our
environment and results from an abnormal proliferation of hematopoietic progenitor cells that
generate alterations in the different cell series.
Objectives: Determine the epidemiological, clinical and laboratory profile at debut of patients
under 15 years of age with a diagnosis of acute lymphoblastic leukemia in the three main hospitals
of the city of Cusco during the years 2018 and 2023.
Materials and methods: Non-experimental, descriptive, cross-sectional, observational and
retrospective study. 92 physical and digitalized medical records were reviewed in the three Cusco
hospitals of patients under 15 years of age with a confirmed diagnosis of acute lymphoblastic
leukemia; The results obtained were presented in tables expressed in absolute frequencies and
percentages.
Results: The case mix of pediatric patients who debuted each year with acute lymphatic leukemia
between 2018 and 2023 was 14 cases per year. Among the most important epidemiological
characteristics we have that the male sex was the most affected (62%), as well as the most
registered age groups were 1 to 5 years and 11 to 15 years with (62%) and ( 40%) respectively.
The place where the patients came from most frequently was the provinces (48%), with the
Convention being the province with the highest number of cases in this group (23%). The most
frequent symptoms at debut were paleness (92%), asthenia/adynamia (80%), general malaise
(79%), bleeding/hemorrhage (45%). On physical examination, the most frequent finding at debut
was lymphadenopathy (70%), of which cervical lymphadenopathy represented (81%). At the
laboratory level it was found that (57%) debuted with severe anemia, of which (73%) was
normocytic/normochromic, at the platelet count level it was evident that (33%) debuted with less
than fifty thousand platelets, (63%) of cases debuted with lymphocytosis, (43%) debuted with
leukopenia and (36%) with severe neutropenia. LDH was found above 500 U/I in (58%) of
patients, and it was also found that (25%) of patients had uric acid levels above 8mg/dl. The most
frequent strain of leukemia was type B with (90%), with the common subtype B being the most
frequent of these with (78%) and other types in (3%), the most frequent karyotype was the diploid
(normal) (58%), and in the molecular panel in (79%) of the cases no alterations were found, all
of this upon the time of the diagnosis.
Conclusions: In the epidemiological characteristics, it was determined that this disease is more
common in males, that the preferred age of debut for this neoplasia is between 1 and 5 years, and
that this disease is more common in the provinces. Clinically, they present with paleness and
lymphadenopathy. In the laboratory findings, the majority of patients presented with severe
anemia, leukopenia, and severe thrombocytopenia, as well as immunophenotype B, diploid
karyotype, and molecular panel without alterations were the most frequent at debut. | en_US |
