Relación entre la condición de ingreso y el pronóstico a los seis meses en pacientes con traumatismo cráneo encefálico en el Hospital Regional del Cusco en el 2022

Objetivo: Determinar la relación entre la condición de ingreso y el pronóstico a los seis
meses en pacientes con traumatismo cráneo encefálico en el hospital Regional del Cusco
en el 2022.
Metodología: Estudio transversal realizado en el Hospital Regional del Cusco durante el
2022. La población total fue de 96 pacientes, de los cuales participaron 93 pacientes con
traumatismo cráneo encefálico que cumplían con los criterios de inclusión como: ser
mayores de 18 años, tener diagnóstico de TCE leve, moderado o severo; se excluyeron 3
por tener datos incompletos. Se calculó el RR obteniendo RR crudos y ajustados por las
variables estadísticamente significativas.
Resultados: de 93 pacientes estudiados, el 81% de los pacientes fue de sexo masculino,
con una media de 46 años. En los factores fisiológicos, encontramos que la frecuencia
respiratoria tuvo una mediana de 20 con un IQR de 4. La presión arterial mediana fue
85.33 con un IQR de 23.33. En la saturación de oxígeno se obtuvo una mediana de 90,
con un IQR de 6. En el puntaje general de Glasgow, se obtuvo una mediana de 13, con
un IQR de 5. En los factores laboratoriales, encontramos que la mediana de tiempo de
Protrombina es 14.5 con un IQR de 2.2. En el INR, encontramos que la mediana fue de
1.18, con un IQR de 0.24. En la glicemia, la mediana fue de 119, con un IQR de 48.
Finalmente, el sodio, con una mediana de 137.5 y u IQR de 10.5. En los factores
tomográficos, encontramos que los pacientes podían tener más de una alteración, como
fractura de cráneo + hemorragia subaracnoidea + desviación de línea media. Observamos
que el 23% presento fractura de base de cráneo, el 12.5% Hemorragia subaracnoidea, el
6% hemorragia Intraparenquimatosa, 25% hemorragia epidural 25%, hemorragia
subdural 23% y desviación de línea media 2.08%. También observamos que la mediana
de horas transcurridas antes de SOP fue de 22.75 horas y un IQR de 30.25. Además,
observamos que el 40% de los pacientes recibieron un tratamiento quirúrgico.
Conclusiones: Existe relación entre factores fisiológicos (Glasgow, saturación de
oxígeno, presión arterial media, frecuencia cardiaca, frecuencia respiratoria), factores
demográficos (edad, sexo, área de procedencia), el tiempo de ingreso a sala de
operaciones y el pronóstico a los seis meses en pacientes con traumatismo
craneoencefálico en el hospital Regional del Cusco en el 2022. No existe relación entre
los factores laboratoriales y tomográficos con el pronóstico a los seis meses en pacientes
con traumatismo craneoencefálico en el hospital Regional del Cusco en el 2022, en el

The total population was 96 patients, of which 93 patients with head trauma

years. In physiological factors, we found that respiratory rate had a median of 20 with an
IQR of 4. Median blood pressure was 85.33 with an IQR of 23.33. In oxygen saturation,
a median of 90 was obtained, with an IQR of 6. In the general Glasgow score, a median
of 13 was obtained, with an IQR of 5. In the laboratory factors, we found that the median
time of Prothrombin is 14.5 with an IQR of 2.2. For the INR, we found that the median
was 1.18, with an IQR of 0.24. In glycemia, the median was 119, with an IQR of 48.
Finally, sodium, with a median of 137.5 and an IQR of 10.5. In the tomographic factors,
we found that patients could have more than one alteration, such as skull fracture +
subarachnoid hemorrhage + midline deviation. We observed that 23% had a skull base
fracture, 12.5% had subarachnoid hemorrhage, 6% had intraparenchymal hemorrhage,
25% had epidural hemorrhage, 25% had subdural hemorrhage, and 23% had midline
deviation. We also observed that the median number of hours elapsed before PCOS was
22.75 hours and an IQR of 30.25. Furthermore, we observed that 40% of patients received
surgical treatment.
Conclusions: There is a relationship between physiological factors (Glasgow, oxygen
saturation, mean arterial pressure, heart rate, respiratory rate), demographic factors (age,
sex, area of origin), time of admission to the operating room and prognosis at six months
in patients with traumatic brain injury at the Regional Hospital of Cusco in 2022. There
is no relationship between laboratory and tomographic factors with the prognosis at six
months in patients with traumatic brain injury at the Regional Hospital of Cusco in 2022,
in the present study.
