dc.description.abstract | Antecedentes: La placenta es la unidad funcional para el crecimiento y supervivencia del
feto es la encargada de brindar todos los requerimientos nutricionales al feto durante el
periodo de gestación. Los procesos patológicos perinatales se presentan por alteraciones
de la función placentaria definido como una insuficiencia placentaria por tal es
indispensable identificar anomalías en su estructura y función. Objetivo: Determinar si
la flujometría doppler de la arteria cerebral media es un buen predictor de insuficiencia
placentaria aguda en fetos a término con crecimiento normal del Hospital Nacional
Adolfo Guevara Velasco-Cusco, periodo 2022 -2023. Método: Este es un estudio de tipo
observacional, prospectivo y transversal. En el cual se incluyó a 98 gestantes de 37 a 42
6/7 semanas de gestación que acataron las normas para ser incluidas y no presentaron
algún criterio de exclusión. Resultados: Se determinó que el riesgo de presentar
indicadores de insuficiencia placentaria en gestantes con un índice de pulsatilidad de la
arteria cerebral media patológico fue para: NST<8 (RR=6.2, IC 3.47 a 11.02), CST<8
(RR=3.7, IC 1.67 a 7.96), APGAR<7 a los 5 minutos (RR=4.6, IC 2.49 a 8.3), categoría
III en el monitoreo intraparto (RR=2.6, IC 1.82 a 3.59), cesárea de emergencia por SFA
(RR=2.1, IC 1.04 a 4.04); además se halló la sensibilidad y especificidad para cada
indicador, encontrándose mayor sensibilidad y especificidad (S:86%, E:97%) como
prueba diagnóstica de istmo aórtico anormal. Conclusiones: Se demostró la utilidad de
la flujometría doppler de la arteria cerebral media como factor predictor la insuficiencia
placentaria aguda en gestantes con fetos a término y postérmino con adecuado
crecimiento, se encontró una buena sensibilidad y especificidad para indicadores de
sufrimiento fetal agudo no severos. La alteración de la flujometría doppler de la arteria
cerebral media en fetos a término y postérmino es un buen factor predictor de
insuficiencia placentaria aguda por tal debería ser tomado en cuenta para el control
temprano de las gestantes con fetos a término o postérmino de adecuado crecimiento antes
de que se produzcan desenlaces adversos. | es_PE |
dc.description.abstract | Background: The placenta is the functional unit for the growth and survival of the fetus
and is responsible for providing all the nutritional requirements to the fetus during the
gestation period. Perinatal pathological processes occur due to alterations in placental
function defined as placental insufficiency, so it is essential to identify anomalies in its
structure and function. Objective: To determine if Doppler flowmetry of the middle
cerebral artery is a good predictor of acute placental insufficiency in full-term fetuses
with normal growth at the Adolfo Guevara Velasco-Cusco National Hospital, period 2022
-2023.Method: This is an observational, prospective, and cross-sectional study. In which
98 pregnant women from 37 to 42 6/7 weeks of gestation who complied with the rules to

sensitivity and specificity for each indicator was found, with greater sensitivity and
specificity (S:86%, E:97%) as a diagnostic test for abnormal aortic isthmus.
Conclusions: The usefulness of Doppler flowmetry of the middle cerebral artery as a
predictor of acute placental insufficiency in pregnant women with term and post-term
fetuses with adequate growth was demonstrated. Good sensitivity and specificity were
found for indicators of non-severe acute fetal distress. The alteration of Doppler
flowmetry of the middle cerebral artery in term and post-term fetuses is a good predictor
of acute placental insufficiency and should therefore be taken into account for the early
control of pregnant women with term or post-term fetuses of adequate growth before of
adverse outcomes occurring. | en_US |
