Evaluación del índice spo2/fio2 y pao2/fio2 como predictores de severidad y mortalidad en pacientes hospitalizados con Covid-19 en el Hospital Regional del Cusco, 2020-2021

Introducción: La COVID-19 se ha convertido en una gran amenaza para la salud a nivel
mundial, por ello se trató de aplicar índices de oxigenación para poder evaluar la
severidad y riesgo de mortalidad en pacientes con COVID-19, dentro de ellos el
SpO2/FiO2 y PaO2/FiO2, sin embargo, estos aún no están bien estudiados en la
enfermedad por SARS-CoV-2.
Objetivo: El objetivo principal es determinar la capacidad predictiva del índice
SpO2/FiO2 y PaO2/FiO2 medidos al ingreso, a las 24 horas y 48 horas de hospitalización,
respecto a severidad y mortalidad en pacientes hospitalizados con COVID-19 en Hospital
Regional Del Cusco, 2020-2021
Materiales y Métodos: Estudio de tipo pruebas diagnósticas de nivel correlacional,
longitudinal, analítico, retrospectivo, de diseño observacional. Se realizó el análisis
estadístico de la curva ROC (AUC), eligiendo el mejor punto de corte mediante el índice
de Youden, del índice SpO2/FiO2 y PaO2/FiO2 para predecir mortalidad y severidad.
Resultados: Se contó con un total de 360 pacientes, de los cuales el 41.94% requirió
intubación, el 37.78% tuvo criterios de ingreso a UCI, finalmente el 36.11% falleció. El
mejor punto de corte para la predicción de intubación fue el medido a las 24 horas (118.1
y 141.6) con AUC de 0.9876 y 0.9893; de igual manera el mejor punto de corte medido
para predicción de ingreso a UCI fue a las 24 horas (88.88 y 136.33) con AUC de 0.9792
y0.9733; finalmente el mejor punto de corte para predicción de mortalidad fue el medido
a las 48 horas (95.00 y 113.75) con AUC de 0.9764 y 0.9678 para el índice PaO2/FiO2 y
SpO2/FiO2 respectivamente. Introduction: COVID-19 has become a great threat to health worldwide, so we tried to
apply oxygenation indices to assess the severity and risk of mortality in patients with
COVID-19, including SpO2. /FiO2 and PaO2/FiO2, however, these are not yet well
studied in SARS-CoV-2 disease.
Objective: The main objective is to determine the predictive capacity of the SpO2/FiO2
and PaO2/FiO2 index at admission, at 24 hours and 48 hours of hospitalization, regarding
severity and mortality in hospitalized patients with COVID-19 in Hospital Regional Del
Cusco, 2020-2021
Materials and Methods: Study of diagnostic test type correlational, longitudinal,
analytical, retrospective, observational design. Statistical analysis of the ROC curve
(AUC) was performed, choosing the best cut-off point using the Youden index, the
SpO2/FiO2 and PaO2/FiO2 index to predict mortality and severity.
Results: There was a total of 360 patients, of which 41.94% required intubation, 37.78%
had ICU admission criteria, finally 36.11% died. The best cut-off point for the prediction
of intubation was the one measured at 24 hours (118.1 and 141.6) with AUC of 0.9876
and 0.9893; Similarly, the best cut-off point measured for prediction of ICU admission
was at 24 hours (88.88 and 136.33) with AUC of 0.9792 and 0.9733; Finally, the best cutoff
point

was

Conclusion: The SpO2/FiO2 and PaO2/FiO2 indices measured at 24 hours are good
predictors of severity and measured at 48 hours are good predictors of mortality.
