dc.description.abstractAntecedentes: Se ha considerado que la tasa ideal para las cesáreas debe oscilar entre 10% y 15%. Ya que no hay indicios que frecuencias por encima de estos valores mejoren las tasas de mortalidad materna y/o neonatal. Según ENDES 2018, la prevalencia de cesáreas en el Perú se incrementó de 22,9% a 34,5% entre los años 2011 y el primer semestre de 2018. Este incremento en la frecuencia de cesáreas trae consigo también el aumento de riesgo de presentar complicaciones, siendo una de las más frecuentes la infección de sitio operatorio post cesárea que ha sido estudiada en diferentes escenarios, donde se han encontrado incidencias desde 0,58% hasta 11%. Lo cual implica una mayor estancia hospitalaria, mayores gastos de atención y un aumento de morbilidad materna, teniendo un impacto aún mayor en países de bajos recursos. Métodos: Se realizó un estudio observacional analítico, transversal, retrospectivo, de tipo casos y controles, teniendo una población objetivo total de 71 pacientes. Para el análisis de los datos, se utilizó el programa estadístico STATA 15.5 y se procedió a determinar la distribución de frecuencias (absolutas y relativas) de las variables cualitativas y de tendencia central (media y mediana) y de dispersión (desviación estándar) para las variables cuantitativas. Para el análisis bivariado y evaluar las diferencias estadísticas se utilizó chi cuadrado y Prueba exacta de Fisher para las variables cualitativas, para la variables cuantitativas se utilizó la prueba T de student y la prueba de Levene. Para la medición de los factores asociados con la infección de sitio operatorio se calcularon Odds Ratios como medida de riesgo de forma bivariada, Resultados: En el presente estudio se analizaron los diferentes factores encontrados en 174 pacientes entre casos (58) y controles (116). En este estudio se observó una asociación estadísticamente significativa entre la infección de sitio operatorio y la corioamnionitis con un ORa de 8,48 (1,46-49,12) p=0,02. La infección de tracto urinario durante el tercer trimestre y el desarrollo de infección de sitio operatorio tuvieron asociación estadísticamente significativa con un ORa de 2,98 (1,16-7,65) p=0,02. A partir de 5 o más tactos vaginales estos se asocian de forma estadísticamente significativa al desarrollo de infección de sitio operatorio con un ORa de 3,84 (1,43-10,32) p=0,01. se observó la existencia de asociación estadísticamente significativa entre el sangrado mayor a 500 ml y la infección de sitio operatorio con un ORa de 4,21 (1,11-15,91) p=0.03. Conclusiones: Se identificó como factor de riesgo la presencia de corioamnionitis, infección de tracto urinario durante el tercer trimestre, sangrado mayor a 500 ml durante el procedimiento y la realización de 5 a más tactos vaginales.es_PE
dc.description.abstractBackground: It has been considered that the ideal rate for caesarean sections should range between 10% and 15%. Since there is no evidence that frequencies above these values improve maternal and / or neonatal mortality rates. According to ENDES 2018, the prevalence of caesarean sections in Peru increased from 22.9% to 34.5% between the years 2011 and the first half of 2018. This increase in the frequency of caesarean sections also brings with it an increased risk of presenting complications, one of the most frequent being post-caesarean section infection of the operative site that has been studied in different settings, where incidences from 0.58% to 11% have been found. This implies a longer hospital stay, higher costs of care and an increase in maternal morbidity, having an even greater impact in low-income countries. Methods: An analytical, cross-sectional, retrospective, case-control, observational study was carried out, with a total target population of 71 patients. For data analysis, the statistical program STATA 15.5 was used and the frequency distribution (absolute and relative) of the qualitative and central tendency variables (mean and median) and dispersion (standard deviation) for the quantitative variables. For the bivariate analysis and to evaluate the statistical differences, chi square and Fisher's exact test were used for the qualitative variables, for the quantitative variables the Student's T test and the Levene test were used. To measure the factors associated with infection at the surgical site, Odds Ratios were calculated as a risk measure on a bivariate basis. Results: In the present study, the different factors found in 174 patients between cases (58) and controls (116) were analyzed. In this study, a statistically significant association between operative site infection and chorioamnionitis was observed with an ORa of 8.48 (1.46-49.12) p = 0.02. Urinary tract infection during the third trimester and the development of infection at the surgical site had a statistically significant association with an ORa of 2.98 (1.16-7.65) p = 0.02. From 5 or more vaginal examinations, these are statistically significantly associated with the development of infection at the operative site with an ORa of 3.84 (1.43- 10.32) p = 0.01. The existence of a statistically significant association was observed between bleeding greater than 500 ml and surgical site infection with an ORa of 4.21 (1.11-15.91) p = 0.03. Conclusions: The presence of chorioamnionitis, urinary tract infection during the third trimester, bleeding greater than 500 ml during the procedure and the performance of 5 or more vaginal examinations was identified as a risk factor.en_US
