Factores pronósticos de supervivencia de cáncer de mama en el Hospital Nacional Adolfo Guevara Velasco de Essalud Cusco, periodo 2008 – 2017.

Abstract

“Factores pronósticos de supervivencia de cáncer de mama en el Hospital Nacional Adolfo Guevara Velasco (HNAGV) EsSalud Cusco, periodo 2008 - 2017”. Introducción: La supervivencia en las pacientes con cáncer de mama varía según el momento del diagnóstico, la edad, el estadio clínico, el patrón histológico, tamaño y localización del tumor. Objetivo: Identificar los factores pronósticos de supervivencia de cáncer de mama en pacientes del HNAGV de EsSalud Cusco. Metodología: Se realizó un estudio de supervivencia retrospectivo. Los casos se obtuvieron del registro de unidad de cáncer del HNAGV. Se hizo análisis por grupos de edad y se calcularon la supervivencia a 5 años. Se utilizó el método actuarial y Kaplan-Meier según variables, con IC de 95 % y p < 0,05 en el programa SPSSv22.0. Resultados: Se analizó los resultados de 354 pacientes, la edad promedio fue 53,3 años, la supervivencia global a 5 años fue de 79 %, el grupo etario de 40 a 49 años tienen una supervivencia de 88,5 %; las que tienen menos de 40 años 77, 8 %. La supervivencia de acuerdo al estadio clínico fueron de 93, 3 % para estadio (0 y I), 88,8% para el estadio II, 77,4% para el estadio III y para el estadio IV de 29% (p <0,0001); La supervivencia en relación al patrón histológico el ductal tiene una supervivencia de 80 % y el mixto 94,1%. Conclusiones: La supervivencia global a 5 años de las pacientes con cáncer de mama en HNAGV, fue de 79%. La supervivencia a 5 años en estadios tempranos (estadios 0 y I) fue de 93,3 % y de 29 % para estadio IV. El estadio clínico 0 y I, el grupo etario de 40 a 49 años y la localización del tumor en el cuadrante inferior interno, son factores pronósticos de mayor supervivencia.

“Prognostic factors of survival of breast cancer in the Adolfo Guevara Velasco National Hospital (HNAGV) EsSalud Cusco, period 2008 - 2017“. Introduction: Survival in breast cancer patients varies according to the time of diagnosis, age, clinical stage, histological pattern, size and location of the tumor. Objective: To identify the prognostic factors of breast cancer survival in HNAGV patients of EsSalud Cusco. Methodology: A retrospective survival study was conducted. The cases were obtained from the cancer unit registry of the HNAGV. Analysis was done by age groups and 5-year survival was calculated. We used the actuarial method and Kaplan-Meier according to variables, with 95% CI and p <0.05 in the SPSSv22.0 program. Results: We analyzed the results of 354 patients, the average age was 53.3 years, the overall survival at 5 years was 79%, the age group from 40 to 49 years have a survival of 88.5%; those who are less than 40 years old 77, 8%. Survival according to clinical stage was 93.3% for stage (0 and I), 88.8% for stage II, 77.4% for stage III and for stage IV 29% (p <0 , 0001). The survival in relation to the histological pattern of the ductal has a survival of 80% and the mixed 94.1%. Conclusions: The 5-year overall survival of patients with breast cancer in HNAGV was 79%. The 5-year survival in early stages (stages 0 and I) was 93.3% and 29% for stage IV. The clinical stage 0 and I, the age group of 40 to 49 years and the location of the tumor in the internal lower quadrant, are prognostic factors of greater survival.
