dc.description.abstract | Antecedentes: La Fibrilación Auricular tiene una prevalencia de 2 a 9% y aumenta cinco veces el riesgo de accidente cerebrovascular. La no adherencia a anticoagulación oral con warfarina se identifica como un determinante importante de un deficiente tratamiento. El objetivo del estudio fue determinar los factores asociados a la no adherencia a anticoagulación oral en pacientes con Fibrilación Auricular del Hospital Adolfo Guevara Velasco 2018.
Métodos: Se realizó un estudio analítico de corte transversal, a través de una muestra de
los pacientes con fibrilación auricular. Se estudió la no adherencia a anticoagulación oral (Test de Morisky modificado de 8 items) y 18 probables factores asociados a ella. Para el análisis de los datos, se utilizó el Odds ratio (OR), la prueba de Chi cuadrado e intérvalos de confianza al 95%; mediante el programa estadístico SPSS versión 24.0.
Resultados: De los 207 pacientes incluidos en el estudio, 125 no fueron adherentes a la anticoagulación oral con warfarina, representando un 60.4%. Los factores asociados fueron: no tener conocimientos previos de la enfermedad (OR=2.45, IC 95% [1.3-4.5], p=0.003), no tener conocimientos previos del tratamiento (OR= 2.84, IC 95% [1.5-5.4], p=0.001), no contar con apoyo familiar y/o de cuidadores (OR=4.56, IC 95% [2.1-9,9], p=0.01), presencia de efectos secundarios de la warfarina (OR=2.22, IC 95% [1.2-4.2], p=0.01), consumo de alcohol (OR=2.12, IC 95% [1.2-3.8], p=0.01) y consumo de tabaco (OR=2.15, IC 95% [1.1-4.6], p=0.03).
Conclusiones: En nuestro estudio se evidenció que el nivel de no adherencia a la anticoagulación oral con warfarina fue de 60.4%. Los factores asociados fueron: el conocimiento deficiente de la enfermedad, el conocimiento deficiente del tratamiento, no contar con apoyo familiar y/o de cuidadores, presencia de efectos secundarios de la
warfarina, consumo de alcohol y consumo de tabaco. | es_PE |
