Factores asociados a la adherencia de la higiene de manos en personal de salud de los hospitales del III Nivel de atención de la región Cusco, 2021

Objetivo: Determinar los factores asociados a la adherencia de la higiene de manos en
personal de salud de los hospitales del III nivel de atención de la Región Cusco, 2021.
Método: Se realizo un estudio de tipo observacional, correlacional, analítico, transversal,
prospectivo. Se estudiaron a 183 trabajadores de salud de dos hospitales de III nivel de
atención de la Región Cusco. Se usó tres instrumentos para la recolección de datos, el primero
fue el instrumento validado con un alfa de cronbach de 0.804 el cual evalúa los factores
personales e institucionales que influyen en una buena adherencia a la higiene de manos, el
segundo instrumento usado fue el elaborado por Organización Mundial de la Salud en el 2009
el cual evalúa la adherencia a la higiene de manos y el tercer instrumento usado fue el formato
de monitoreo de la técnica de higiene de manos elaborada por la Dirección de Gestión y
Calidad de la Gerencia Regional del Cusco el cual evalúa el cumplimiento de los 11 pasos y
el tiempo que implica la técnica de higiene de manos. Se uso el calculo de frecuencia absoluta
y relativa para las variables cuantitativas, así como, las medidas de tendencia central y
dispersión para las variables cualitativas; asimismo; se aplico el análisis bivariado para
determinar la asociación entre variables usando el Chi cuadrado; se utilizo regresión logística
para obtener los ORa los que fueron hallados en el programa estadístico SPSS v.25.0 para
macOs.
Resultados: Se evaluó 183 trabajadores de salud obteniendo una adherencia en 114
trabajadores de salud (62.3%). Los factores asociados mas importantes hallados son el
departamento de trabajo donde se vio que el departamento Pediatría – Neonatología obtuvo
la mayor adherencia con 44 trabajadores de salud (38.6%, ORa 1.605 <0.000, IC 95%), el
grupo ocupacional con mayor adherencia fue el de enfermería con 38 enfermeras (33.3%,
ORa 1.285, p=0.006, IC 95%). Otros factores asociados fueron la edad entre 41 a 45 años
con 24 trabajadores de salud (21.1%; ORa ,p=0.006, IC 95%), años de servicio entre 5 a 10 años con 49 trabajadores de salud (43.0%, ORa 1.755, p=0.000, IC 95%), conocimiento a los
5 momentos de la higiene de manos para la atención con 95 trabajadores de salud (83.3% ,
ORa 3.625, IC 95%; 1.825 – 7.201), conocimiento de la técnica de la higiene de manos con
92 trabajadores de salud (80.7% , ORa 2.091, IC 95%; 1.056 – 4.141), alergias con 82
trabajadores de salud (71.9%, ORa 3.141, IC 95%; 1.680 – 5.874), ambiente físico con 104
trabajadores de salud (91.2%, ORa 3.671, IC 95%; 1.581 – 8.524), capacitación con 91
trabajadores de salud (79.8%, ORa 4.316, IC 95%; 2.236 – 8.330), recursos materiales 123
trabajadores de salud (78.1%, ORa 3.665, IC 95%; 1.917 – 7.004).
Conclusiones: Existe una correlacion entre los factores personales e institucionales (años de
servicio, conocimiento de los 5 momentos de la higiene de manos para la atención,
conocimiento de la técnica de higiene de manos, alergias, edad, grupo ocupacional, ambiente
físico adecuado, capacitación, recursos materiales y departamento donde trabajan) y una
adecuada adherencia a la higiene de manos en el personal de salud de los hospitales del III
nivel de atención de la Región Cusco. Objectives: Determine the factors associated with adherence to hand hygiene in health
personnel of the hospitals of the III level of care in the Cusco Region, 2021.
Methods: An observational, correlational, analytical, cross-sectional, prospective study was
carried out. A total of 183 health workers from two level III hospitals in the Cusco Region
were studied. Three instruments were used for data collection, the first was the validated
instrument with a Cronbach's alpha of 0.804 which evaluates the personal and institutional
factors that influence good adherence to hand hygiene, the second instrument used was the
elaborated by the World Health Organization in 2009 which measures adherence to hand hygiene and the third instrument used was the Hand Hygiene Technique Monitoring Form
prepared by the Management and Quality Directorate of the Regional Management of Cusco
which evaluates compliance with the 11 steps and the time involved in the hand hygiene
technique. The calculation of absolute and relative frequency was used for quantitative
variables, as well as measures of central tendency and dispersion for qualitative variables; in
addition; bivariate analysis was used to determine the association between variables using
the Chi square; Logistic regression was used to obtain the ORa which were found in the
statistical program SPSS v.25.0 for macOS.
Results: 183 health workers were evaluated, obtaining adherence in 114 health workers
(62.3%). The most important associated factors found are the work department where it was
seen that the Pediatrics - Neonatology department obtained the highest adherence with 44
health workers (24.0%, ORa 1.605 <0.000, CI 95%), the occupational group with the highest
adherence was the nursing one with 38 nurses (20.8%, ORa 1.285, p=0.006, CI 95%). Other
associated factors were age between 41 and 45 years with 24 health workers (13.1%; ORa,
p=0.006, CI 95%), years of service between 5 and 10 years with 49 health workers (26.8%,
ORa 1.755 , p=0.000, CI 95%), knowledge at 5 moments of hand hygiene for care with 95
health workers (51.9%, ORa 3.625, CI 95%; 1.825 - 7.201), knowledge of the technique of
hand hygiene with 92 health workers (50.3%, ORa 2.091, CI 95%; 1.056 - 4.141), allergies
with 82 health workers (44.8%, ORa 3.141, CI 95%; 1.680 - 5.874), physical environment
with 104 health workers (56.8%, ORa 3.671, CI 95%; 1.581 – 8.524), training with 91 health
workers (49.7%, ORa 4.316, CI 95%; 2.236 – 8.330), material resources 123 health workers
(48.6% , ORa 3.665, CI 95%, 1.917 – 7.004).
Conclusions: There is a correlation between personal and institutional factors (years of
service, knowledge of the 5 moments of hand hygiene for care, knowledge of hand hygiene
technique, allergies, age, occupational group, adequate physical environment, training,
material resources and department where they work) and adequate adherence to hand hygiene
in the health personnel of the hospitals of the III level of care in the Cusco Region.
