Caracterización del asma en altura en niños de 05 -14 años de edad en el Hospital Adolfo Guevara Velasco de la ciudad del Cusco, 2020

Introducción: La OMS reconoce al asma como una enfermedad no transmisible
común entre los niños, según sus estimaciones hubo 383 000 muertes por asma en el 2015,
convirtiendo a esta entidad patológica en tema crucial de salud pública a nivel mundial.
Actualmente en el Perú la adquisición de información actualizada sobre asma en cuanto a
prevalencia y bases sólidas sobre factores de riesgo es escasa. Los estudios existentes
además de ser limitados, se centran principalmente en la evaluación y análisis de zonas
urbanas dejando de lado otras áreas del país.
Objetivo: Identificar las características de asma en niños de 5-14 años de edad en el
hospital Adolfo Guevara Velasco de la ciudad del Cusco, 2020.
Método: Estudio descriptivo, transversal y retrospectivo de serie de casos. Con muestra de
131 niños entre 05 y 14 años de edad con diagnóstico de asma del Hospital Adolfo Guevara
Velasco de la ciudad del Cusco que cumplieron con los criterios de inclusión.
Resultados: Se realizó el estudio en 131 niños. En características generales existe
predominio del sexo masculino en un 55%, y la mayoría están entre los 05 a 08 años de edad.
En las características prenatales se observa que los antecedentes maternos y paternos de asma
están en el 11.5% IC 95% [6-16.9] y 19.1% IC 95% [12.35-25.81] respectivamente. El
antecedente materno de rinitis alérgica se presentó en un 17.6% IC 95% [11.04-24.07] y el
paterno en un 21.4% IC 95% [14.35-28.39]. El antecedente materno de dermatitis atópica se
aprecia en un 26.7% IC 95% [19.14-34.29], y el paterno en un 19.1% IC 95% [12.35-25.81].
Un 5.3% IC 95% [1.49-9.19] de las madres presentó tabaquismo materno durante la
gestación. El 42% del total de madres utilizaron antibióticos durante la gestación. En las
características natales, se realizó cesáreas al 38.2% de madres, el nacimiento prematuro se
presentó en el 16.8% IC 95% [10.39-23.20] de los niños. El 13% IC 95% [7.22-18.73] de
niños presentó bajo peso al nacer. En las características postnatales encontramos que el uso
de fórmulas se dio en el 70.2% del total, siendo este antes de los 4 meses en un 55.7% IC
95% [47.22-64.23], y un consumo desde los 4 meses en un 14.5% IC 95% [8.47-20.53] de
niños. La lactancia materna exclusiva fue ofrecida al 44.3% de los niños. En el 51.9% se
brindó lactancia materna hasta los 8 a 12 meses y los que recibieron hasta los 24 meses o
más son el 10.7% IC 95% [5.4-15.98]. El consumo de leche de vaca y sus derivados en la
dieta se presenta a diario en el 25.2% IC 95% [17.76-32.62] de los niños. El consumo de
leche de vaca en sus diferentes presentaciones el primer año de vida se observó en el 70.2%
IC 95% [62.4-78.06] de los niños. El consumo de huevo y pescado el primer año de vida predominó en un 45%. La alimentación complementaria inició en el 51.9% a los 6 meses, y
el 38.2% iniciaron a los 8 meses o después. El 51.1% de los niños tuvo presencia de mascotas
como perro y/o gato el primer año de vida. La exposición a tabaco el primer año de vida por
su parte se observó en el 9.9% IC 95% [4.80-15.04] de los niños. El 51.9% de la población
de estudio presentó ictericia neonatal, del cual un 30.5% IC 95% [22.65-38.42] recibió
tratamiento con fototerapia. El diagnóstico actual de dermatitis atópica se presentó en el
63.4% de los niños, y rinitis alérgica en el 61.1%. El 54.2 % de los niños presentó el
antecedente de bronquiolitis. El uso de antibióticos en el primer año de vida se realizó en el
54.2%. En la caracterización del asma respecto a los criterios del IPA se observa una baja
incidencia de antecedentes familiares de asma, las sibilancias no asociadas a resfrío antes de
los 3 años estuvo presente en el 67.9% IC 95% [59.95-75.93] de los niños, las Sibilancias
frecuentes más de 3 episodios en un año antes de los 3 años en un 49.6% IC 95% [41.0658.18].El
recuento
de
Eosinófilos
en
sangre
periférica
≥4%
se
aprecia
en
el
58%
IC
95%
[49.56-66.47].
Y para finalizar, la alergia a proteína de leche, huevo, frutos secos está
presente en el 26.7% IC 95% [19.14-34.29] de los niños participes del estudio.
Conclusiones: En la caracterización del asma en altura hay ausencia de antecedentes
marcados de asma, rinitis alérgica y dermatitis en los padres. El parto por cesárea no se
observó en la mayoría de pacientes, así como tampoco el bajo peso al nacimiento. El uso de
fórmulas lácteas predomina en la población de estudio, siendo este consumo mayor antes de
los 04 meses de vida. La mayor parte de niños asmáticos no recibió lactancia materna
exclusiva y el tiempo mayoritario que recibió lactancia fue hasta los 08 a 12 meses. El
consumo de leche de vaca y sus derivados en la dieta a diario es frecuente. El inicio de
alimentación complementaria se realizó a los 6 y 8 meses a más en niños asmáticos. Se
observa que la mayoría de niños presentó ictericia neonatal de los cuáles varios recibieron
fototerapia. Presentan diagnóstico de dermatitis atópica y rinitis alérgica, además del
antecedente de bronquiolitis. El uso de antibióticos el primer año de vida es frecuente. Introduction: The WHO recognizes asthma as a common non-communicable
disease among children, according to its estimates there were 383,000 asthma deaths in
2015, making this pathological entity a crucial public health issue worldwide. Currently in
Peru the acquisition of updated information on asthma regarding prevalence and solid bases
on risk factors is scarce. The existing studies, in addition to being limited, focus mainly on
the evaluation and analysis of urban areas, leaving aside other areas of the country.
Objective: To identify the characteristics of asthma in children aged 5-14 years at
the Adolfo Guevara Velasco hospital in the city of Cusco, 2020.
Method: Descriptive, cross-sectional and retrospective study of case series. With a
sample of 131 children between 05 and 14 years of age with a diagnosis of asthma who met
the inclusion criteria of the Adolfo Guevara Velasco Hospital in the city of Cusco.

In

postnatal
characteristics
we
found
that
the
use
of
infant
formulas
occurred
in

children. Exclusive breastfeeding was offered to 44.3% of the children. In 51.9%,
breastfeeding was provided up to 8 to 12 months, and those who received up to 24 months
or more are 10.7% 95% CI [5.4-15.98]. Consumption of cow's milk and its derivatives in
the diet occurs daily in 25.2% 95% CI [17.76-32.62] of children. Consumption of cow's
milk in its different presentations in the first year of life was observed in 70.2% 95% CI
[62.4-78.06] of children. The consumption of egg and fish in the first year of life
predominated in 45%. Complementary feeding started in 51.9% at 6 months, and 38.2%
started at 8 months or later. A 51.1% of children had pets such as dogs and / or cats in the
first year of life. Exposure to tobacco in the first year of life was observed in 9.9% 95% CI
[4.80-15.04] of children. 51.9% of the study population presented neonatal jaundice, of
which 30.5% 95% CI [22.65-38.42] received treatment with phototherapy. The diagnosis
of atopic dermatitis was presented in 63.4% of children, and allergic rhinitis in 61.1%.
54.2% of the children had a history of bronchiolitis. The use of antibiotics in the first year
of life was carried out in 54.2%. In the characterization of asthma with respect to the IPA
criteria, a low incidence of a family history of asthma is observed, wheezing not associated
with a cold before 3 years was present in 67.9% 95% CI [59.95-75.93] of children Frequent Wheezing more than 3 episodes in one year before 3 years by 49.6% 95% CI [41.06-58.18].
Eosinophil count in peripheral blood ≥4% is appreciated in 58% 95% CI [49.56 -66.47].
And finally, allergy to milk, egg, and nut protein is present in 26.7% 95% CI [19.14-34.29]
of the children participating in the study.
Conclusions: In the characterization of asthma in height there is an absence of a marked
history of asthma, allergic rhinitis and dermatitis in the parents. Cesarean delivery was not
observed in the majority of patients, nor was low birth weight. The use of milk formulas
predominates in the study population, with this consumption being greater before 04
months of age. The majority of asthmatic children did not receive exclusive breastfeeding
and the majority time that they received breastfeeding was up to 08 to 12 months. The
consumption of cow's milk and its derivatives in the diet on a daily basis is frequent.
Complementary feeding was started at 6 and 8 months later in asthmatic children. It is
observed that the majority of children presented with neonatal jaundice, of which several
received phototherapy. They present a diagnosis of atopic dermatitis and allergic rhinitis,
in addition to a history of bronchiolitis. The use of antibiotics in the first year of life is
frequent.
